Medicare Coverage
At Fit My Feet Orthotics & Shoes, we proudly help patients navigate Medicare coverage for diabetic footwear and orthotics.
If you qualify, Medicare may assist in covering the cost of shoes and inserts designed to improve comfort, reduce pressure points, and help prevent foot complications related to diabetes.
Please Note:
We currently work with Medicare only. We do NOT process Medicaid coverage.
Eligibility Requirements
To be considered for Medicare assistance, you must meet the following criteria:
• Be 65 years of age or older and enrolled in Medicare
• Have a diagnosis of diabetes
• Have at least one additional qualifying foot condition such as neuropathy, plantar fasciitis, heel spurs, poor circulation, or other diabetes-related foot complications
• Have your physician verify and certify your condition through the required documentation
Medicare under a PPO secondary plan may assist in covering the cost of diabetic shoes and orthotics. Coverage varies by individual plan and is not guaranteed.
Required Documentation
In order to begin the Medicare qualification process, your physician must complete and sign the required form.
Statement of Certifying Physician (SOCP)
This form confirms that you have diabetes and a qualifying foot condition.
Please download the SOCP form above and bring it to your doctor so they can complete and sign it. Once the form has been completed, return it to Fit My Feet Orthotics & Shoes so our team can review the documentation and determine the next steps.
